Perhaps this explains why it happens--they need protocol to tell them to unload it. It is for the same reason that I get nervous in restaurant bathrooms when I see a sign that says "Employees must wash hands..."

I'm not a gambling man, but I'd bet the .45 caliber in question is a Glock 21, which you have to press the trigger to disassemble.
Not the gun's fault. Too bad he has to learn safe gun handling the hard way. I hope he wasn't seriously hurt and will receive appropriate remedial training.
